Interventionelle Therapie der benignen Prostatahyperplasie Übersicht und Wertung

Interventional treatment of benign prostatic hyperplasia - summary and evaluation Hinsichtlich ihrer Effektivität gilt die transurethrale Prostataresektion (TUR-P) bei der symptomatischen benignen Prostatahyperplasie (BPH) trotz zahlreicher instrumenteller Alternativen weiterhin als Goldstandard der operativen Therapie. Aus der vergleichsweise unverändert hohen postoperativen Komplikationsrate von ca. 18% und einer Reoperationsrate von 12-15% innerhalb der ersten acht Jahre resultiert das immense Bestreben zur Entwicklung der alternativen Techniken. Die Methoden der transurethralen Prostatainzision (TUI-P) transurethralen Vaporisation der Prostata (TUV-P) der Laserbehandlungsverfahren (ILK TULIP TULAP Laser-TUR) der transurethralen Nadelablation der Prostata (TUNA) der fokussierten Ultraschallbehandlung (HIFU) der transurethralen Ballondilatation der transrektalen Hyperthermie der transurethralen Mikrowellen-Thermotherapie (TUMT) und der intraurethralen Implantate (Stents) werden kurz vorgestellt und in ihrer Effektivität mit den etablierten Techniken (TUR-P und offene Prostataadenomektomie) verglichen. Abstract Despite numerous instrumental alternatives transurethral resection of the prostate (TUR-P) is still considered to be the state-of-the-art method for surgical therapy in case of symptomatic benign prostatic hyperplasia. The immense demand for the development of alternative techniques emanates from a comparatively steady high rate of postoperative complications of approximately 18% and a rate of repeated surgery of 12-15% within the first eight years. The methods of transurethral incision of the prostate (TUI-P) transurethral vaporization of the prostate (TUV-P) laser therapy (interstitial laser coagulation of the prostate TULIP TULAP Laser-TUR) transurethral needle ablation of the prostate (TUNA) focussed ultrasound therapy (HIFU) transurethral balloon dilatation transrectal hyperthermia transurethral microwave therapy (TUMT) and urethral stents are briefly presented. Their effectiveness is compared with the established techniques (TUR-P and transvesical prostatectomy).
