Multimorbidität und Polypharmazie als Ursache von Verwirrtheitszuständen

Multimorbidity and polypharmacy as causes of delirium in the elderly Verwirrtheitszustände (VZ) können bei alten Patienten lebensbedrohlich sein und bedürfen daher umgehender diagnostischerAbklärung und Therapie. Sie stehen häufig im Zusammenhang mit der alterstypischen Multimorbidität und einer damit oft einhergehenden Polypharmazie. Unspezifische Alterungsprozesse strukturelle Hirnerkrankungen und der Verlust der homöostatischen Regulationsfähigkeit machen das Gehirn alter Menschen vulnerabel für akute und chronische somatische Krankheiten Nebenwirkungen von Medikamenten selbst in therapeutischen Dosen und sensorische Einflüsse. Daraus folgt daß bei verwirrten Patienten eine sorgfältige Fremdanamnese inklusive Medikamentenanamnese eine Basis-Labordiagnostik und einschlägige bildgebende Verfahren zur diagnostischen Abklärung erforderlich sind. Entscheidend für die Therapie ist an die Möglichkeit eines durch eine organische Grunderkrankung oder durch Medikamente ausgelösten VZ zu denken. Abstract Delirium in the elderly is a life-threatening acute or subacute state of confusion requiring immediate diagnosis and treatment. It is frequently associated with agerelated multimorbidity and polypharmacy. Nonspecific alterations of old age structural cerebral lesions and the reduced capacity for homeostatic regulation make the brain in the very old particularly prone to delirium. Hence delirium in the elderly can be associated with almost any physical illness drugs even in therapeutic doses and sensory deprivation or overload. Organic causes of delirium include fluid and electrolyte disturbances infections intoxications with almost any drug or withdrawal of drug treatments metabolic derangements cerebral ischemia or intracerebral lesions. Therefore a careful history particularly concerning drugs basal laboratory tests and imaging procedures are required in the diagnosis and treatment of patients with delirium. Most important for adequate treatment is simply to think of an organic or drug-related cause of delirium. Although the strongest risk factors for delirium (age and dementia) are not correctable delirium may be prevented by considering a few simple items. These include sufficient fluid supply restriction of the number of drugs to not more than three or four reduction of drug doses according to the altered metabolism of the elderly making use of non-pharmacologic treatments and intensive interdisciplinary communication.
