Berner Klinik für Psychiatrie und Psychosomatik Abteilung für Psychiatrie und Psychotherapie Universitätsklinikum Freiburg Welcher Patient profitiert von einer oralen Pharmakotherapie? Unter einer vollständigen erektilen Dysfunktion leiden ca. 5% der 40jährigen und 15% der 70jährigen Männer. Die standardisierte Anwendung diagnostischer Methoden ermöglicht heute eine ätiologische Klassifizierung der Erektionsstörung. Ebenso wie die Diagnostik erfolgt auch die Therapie nach einem Stufenplan wobei die durch adäquate Diagnostik herausgearbeitete zugrundeliegende Störung die Erfolgsrate der jeweiligen Therapieform und die Präferenz des Patienten/Paares maßgeblich sind. Die Behandlung baut sich von nicht-invasiven zu invasiven Formen auf wobei die heute verfügbare orale Pharmakotherapie mit Sildenafil eine zentrale Rolle spielt. Es ist zu erwarten daß alle therapeutischen Optionen ihren Stellenwert behalten auch wenn sie in der Rangfolge im Stufenplan einen anderen Platz einnehmen. Which Patient may benefit from oral pharmacotherapy? 5% of 40-year-old and 15% of 70year-old men suffer from a complete erectilc dysfunction. A standardized diagnostic workup allows differentiation between the different etiologies. The therapy of erectile disorders follows an algorithmic step-by-step procedure that is guided by the underlying etiology the success rate of the particular therapy and the preference by the Patient or the couple respectively. Adequate therapy proceeds from non-invasive to more invasive interventions. In the near future the newly available oral pharmacotherapy with sildenafil will play a central role. It is expected that all the other therapeutic options will remain important in the treatment of erectile dysfunction even if they take another place in the algorithmic approach.
