International Stem Cell Project in Autoimmune Diseases - Update

Abstract: Department of Rheumatology University of Basel In the past 3 years and for the first time over 80 patients have received a bone marrow transplantation (BMT) as treatment of an autoimmune disease (AD). Most of these were autologous peripheral stem cell and 72 are registered in the EULAR/EBMT data base. 31 patients had multiple sclerosis (MS) and most received BEAM and ATG. as conditioning. 37 patients had rheumatological disorders: 14 scleroderma group 11 RA 4 JCA (systemic) 4 SLE 3 cryoglobulinaemia and 1 Wegeners granulomatosis. Five patients had cytopenias the majority ITP. Toxicity: Four treatment related deaths: infection (aspergillosis influenza pneumonia and septicaemia) and one cerebral haemorrhage. No disease exacerbation during mobilisation with G-CSF. Results: MS - around half the patients improved function (E.D.S.S.). SSC - some patients showed marked improvement including skin score and pulmonary hypertension. One fatal relapse after 6 month improvement. Other: Tendency to improvement or stabilisation. Conclusions: Suggestion of improvement in some therapy resistant AD. Further comparative studies are justified. Toxicity of the procedures similar to other groups e. g. lymphoma. Central data collection essential. It is to early to draw firm conclusions concerning optimal conditioning regimens need for ex-vivo T- and other cell purging or long term benefits. le
