Wirksamkeit teilstationärer Rehabilitation aus Patientensicht Ergebnisse der Evaluation des teilstationären Modellprojektes der Fachklinik Ichenhausen

ERP Hopfen am See 1m&i-Klinikgruppe Enzensberg Hopfen am See 2Fachklinik Ichenhausen Ichenhausen Zusammenfassung: Die teilstationäre Rehabilitation ist eine relativ neue Form der Rehabilitation und beinhaltet - im Gegensatz zur ambulanten Form - das gesamte Angebot der vollstationären medizinischen Rehabilitation mit Ausnahme der Unterbringung und der Vollverpflegung. Teilstationäre Rehabilitation ist mit hohen Erwartungen belastet: Sie soll weniger kosten als eine vergleichbare stationäre Maßnahme aber qualitativ gleichwertig sein. Bisher liegen allerdings kaum empirische Erkenntnisse über diesbezügliche Effekte der teilstationären Rehabilitation vor. Ziel des Beitrags ist die Beantwortung der Frage ob die Ergebnisqualität des neuen Modells aus Patientensicht dem klassischen vollstationären Angebot vergleichbar ist. Mit der Methode der schriftlichen Befragung wurden in zwei unverbundenen Stichproben (teilstationär vollstationär) die notwendigen Daten bei ehemaligen orthopädischen Patienten der Fachklinik Ichenhausen (Bayern) erhoben. Ausgewertet werden konnten 205 Patientenfragebogen (Rücklaufquote: 57%). Es zeigte sich eine hohe Übereinstimmung in der Erfolgsbewertung im subjektiven Gesundheitsempfinden und in der Patientenzufriedenheit zwischen beiden Gruppen. Aus Patientensicht fällt der Rehabilitationserfolg für teilstationäre Patienten bei der Mehrzahl der betrachteten Variablen sogar etwas besser aus als für die stationäre Vergleichsgruppe. Da keines der Ergebnisse statistische Signifikanz erreicht dürfen diese geringfügigen Differenzen aber nicht überbewertet werden. Zwar stellt somit die teilstationäre Rehabilitation eine qualitativ gleichwertige Alternative zum traditionellen vollstationären Angebot dar. Vor allzu großen Erwartungen aber muß gewarnt werden: Teilstationäre Rehabilitation ist keine grundsätzliche Alternative zum stationären Angebot da sie nur für einen kleinen Prozentsatz orthopädischer Patienten überhaupt in Frage kommt und somit vollstationäre Rehabilitation nicht in größerem Umfang ersetzen kann. Sie trägt jedoch zweifellos zu einer flexibleren und bedarfsgerechteren Angebotsstruktur im Rehabilitationswesen bei und kann insofern als wertvolle Ergänzung zur vollstationären und ambulanten Form der Rehabilitation gesehen werden. A Patient Perspective of Partial Hospitalization Rehabilitation - Evaluation Findings of the Partial Hospitalization Model Project at Fachklinik Ichenhausen: Partial-hospitalization rehabilitation is a relatively recent form of rehabilitation which as opposed to the ambulatory approach comprises the entire programme available for inpatient medical rehabilitation except for accommodation and full board. Great expectations are attached to partial hospitalization: It is supposed to cost less than a comparable in-patient measure but matching in terms of quality. However hardly any empirical data exist so far concerning the effects partial hospitalization rehabilitation is having in these respects. The present article seeks to answer the question of whether from a patient perspective the outcome quality of this new model is comparable to that of the classical full-hospitalization programmes. In terms of methodology questionnaires were used to raise the necessary data from two independent samples (i. e. partial vs. full hospitalization) of former orthopaedic rehabilitees of Fachklinik Ichenhausen (Bavaria). Data analysis included some 205 patient questionnaires (57% return rate). High agreement was found among the two groups concerning outcome evaluation subjective health and patient satisfaction. From a patient perspective rehabilitation outcome on the majority of variables considered even is judged slightly better for partial hospitalization patients than for the inpatient comparison group. As none of these results reach statistical significance these minor differences should however not be overrated. While partial-hospitalization rehabilitation hence has turned out an option which matches traditional fullhospitalization programmes in terms of quality too great expectations nevertheless are unwarranted: Partial - hospitalization rehabilitation is no general alternative to inpatient programmes as it will realistically be suitable for a small percentage of orthopaedic patients only hence be unable to replace fullhospitalization programmes to any major extent. Undoubtedly however it is apt to contribute to greater flexibility and needs orientation among the services and programmes available in rehabilitation and may thus be considered a valuable approach for complementing inpatient and ambulatory forms of rehabilitation.
