Abstract: Prof. Dr. med. U. T. Egle (Klinik für Psychosomatische Medizin und Psychotherapie) Dr. med. R. Schwab (Schmerzambulanz der Klinik für Anästhesiologie) Johannes Gutenberg-Universität Mainz. Nach einer hinsichtlich des daraus resultierenden ärztlichen Handelns kritischen Bewertung des Reiz-Reaktions-Konzeptes werden die Grundprinzipien eines bio-psycho-sozialen Schmerzverständnisses und seine Möglichkeiten für ein umfassendes diagnostisches Verständnis chronisch Schmerzkranker skizziert. Als schmerzmodulierende psychosoziale Faktoren werden Aufmerksamkeit bzw. Ablenkung Angst und Depression katastrophisierende Coping-Strategien und fatalistische Krankheitsattribuierung körperliche Mißhandlung und sexueller Mißbrauch in der Kindheit sowie kulturelle Faktoren dargestellt. Als Konsequenzen werden enge fachübergreifende Kooperationsstrukturen für die Versorgung von Patienten mit chronischen Schmerzen postuliert. Abstract Pain und Psyche: Following a critical assessment of the stimulus-response concept and the resulting medical actions the author examines the fundamental principles of bio-psycho-social understanding of pain and the options it offers for comprehensive diagnostic understanding of chronic pain patients. Pain-modulating psychosocial factors such as attention distraction anxiety and depression catastrophising coping strategies and fatalistic illness attribution physical and sexual abuse in childhood as well as cultural factors are discussed. Consequent closely-meshed co-operation structures spanning different disciplines are postulated for the care of chronic pain patients.
