Homehospital für Krebskranke im fortgeschrittenen Stadium: Kosten und Nutzen

Abstract: Prof. Dr. Dr. Stephan Tanneberger Berlin. 1Associazione Nazionale Tumori Bologna Italy 2Divisione Oncologia Medica Azienda Ospedaliera Sant'Orsola Malpighi Bologna Italy A home hospital for progressive tumor diseases - costs and benefit Zusammenfassung Im Bologna Homehospital (BHH) wurden bis zum 30. 06. 1996 insgesamt 15 290 Patienten mit fortgeschrittenen Tumorleiden betreut. Derzeit liegen die Kosten des BHH im Mittel bei 118 798 Lire pro Betreuungstag (range 108 569-129 027 Lire in Abhängigkeit von der Pflegestufe). Bei vergleichsweise niedrigen Kosten des BHH kann eine hohe Betreuungsintensität der Patienten realisiert werden. 98% der Patienten waren mit ihrer Lebensqualität zufrieden. Im Vergleich des BHH (134 Patienten) mit einer stationären Einrichtung (102 Patienten Divisione Oncologia Medica Azienda Ospedaliera Sant' Orsola Malpighi Bologna) wurden bezüglich Nachtschlaf Mahlzeiten und Familienkontakt deutliche Vorteile beim BHH gefunden. Signifikant weniger Patienten des BHH gaben eine reduzierte oder schlechte Lebensqualität (51 % vs. 67%) an oder wünschten den Übergang in die alternative Betreuungsform (03% vs. 47%). Bei der Begründung für die sicher wünschenswerte schrittweise Einführung hochentwickelter ambulanter Betreuungsmodelle wie das Homehospitals für Krebskranke im fortgeschrittenen Stadium sollte in erster Linie auf deren Bedeutung für die Lebensqualität der Kranken hingewiesen werden. Dabei kann es nicht darum gehen einen Prioritätsanspruch häuslicher Betreuung zu begründen. Das letzte Wort über "seine Lebensqualität" spricht immer der Patient. Abstract 15 290 patients have been treated in the Bologna homehospital (BHH)until June 30 1996. The average daily costs in BHH were estimated as 118789 Liras (ranging from 108 569-129 027 Lire depending on the nursing category). Care intensity and patient's quality of life in the BHH are high. 98% of patients were content with the setting in which they were nursed. A questionaire on the degree of satisfaction with the care was completed by 134 BHH patients and 102 patients of Divisione Oncologia Medica Azienda Ospedaliera Sant' Orsola Malpighi Bologna. Satisfaction with respect to sleeping meals and family communications was expressed more often by BHH patients. Less patients of the BHH evaluated "quality of life" reduced or bad (51 % vs. 67%) or requested a transfer to the alternative setting (03% vs. 47%). Advocating step by step introduction of home care quality of life aspects have priority. Certainly home care deserves greatest attention providing care during the life with cancer. However the final decision about the settings of nursing has to be made by the patients themself in accordance with his understanding of quality of life.
