Psychosomatische Rehabilitation bei Störungen am Bewegungsorgan - Erkundungsstudie zu einem indikativen Therapieprogramm

Abstract: Roland-Klinik Bad Meinberg Dipl.-Psych. Eberhard Schneider Horn-Bad Meinberg Zusammenfassung: Patienten mit Beschwerden im Bereich des Bewegungsapparats und Diagnosen aus dem Bereich der neurotischen Störungen und der Somatisierungsstörungen attribuieren ihre Krankheit häufig rein organbezogen so daß nur ein Teil dieser Patienten für eine konfliktorientierte Gruppenpsychotherapie offen ist. Den unterschiedlichen Behandlungsbereitschaften der Patienten ist in der stationären Rehabilitation daher durch ein differenziertes Behandlungskonzept mit psychosomatischem Basisprogramm und fakultativer konfliktorientierter Gruppenpsychotherapie Rechnung zu tragen. In der Roland-Klinik in Bad Meinberg wird ein solches Modell integrierter Kurzzeitpsychotherapie praktiziert. In einer 1993/94 durchgeführten Erkundungsstudie untersuchten wir das Verhältnis zwischen dem anfänglichen auf das Psychische bzw. das Somatische bezogenen Leidensdruck der Patienten und der Teilnahme an den unterschiedlichen Behandlungsmodalitäten. Der anfängliche Leidensdruck erwies sich als Prädiktor für die Teilnahme an der Psychotherapiegruppe. Eine Reihe bedeutsamer Einflüsse auf die Wahl der Behandlungsmodalität und den Therapieerfolg wird diskutiert. Die Ergebnisse der Studie sprechen für eine Fortentwicklung des bestehenden differenzierten Behandlungsansatzes. Psychosomatic Rehabilitation and Disorders of the Musculoskeletal System - An Exploratory Study: Patients suffering from complaints of the musculoskeletal system with diagnoses like neurotic and somatization disorders often attribute their complaints to mere organic factors. Only part of these patients are willing to participate in conflict-orientied group psychotherapy. Taking account of the different treatment motivations in inpatient rehabilitation it is necessary to install a differentiated treatment concept with a basic psychosomatic programme and optional group psychotherapy. At the Roland Klinik in Bad Meinberg such a model of integrated short term psychotherapy has been established. In a 1993-94 pilot study we explored the relationship between the initial subjective suffering (in psychic and organic terms) and the use of the different treatment modalities. The degree of suffering from psychic problems emerged as a predictor for the patient's choice of the psychotherapy group. Some relevant factors for the choice of treatment modalities and outcome of therapy are discussed. The findings of this study support further refinement of the existing treatment differentiation.
