Dermatologische Klimatherapie - Definition Indikationen und gesundheitspolitische Notwendigkeit

Elisabeth Vocks Dermatologische Klinik und Poliklinik der Technischen Universität München München Dermatological Climatotherapy - Definition Indications and Health-political Necessity Zusammenfassung Die dermatologische Klimatherapie wird zur Behandlung chronischer bzw. chronisch-rezidivierender Langzeitdermatosen wie insbesondere Neurodermitis constitutionalis atopica und Psoriasis in Ergänzung zur dermatologischen ambulanten oder stationären Therapie am Wohnort eingesetzt. Für die klassisch-dermatologische stationäre Therapie in ausgewählten erwiesenermaßen therapeutisch günstigen starken Reizklimaten kommen hauptsächlich das Nordseeinselklima und das Hochgebirgsklima in bestimmten Hochgebirgstallagen über 1 500 Meter ü. NN wie insbesondere im Hochtal von Davos in Frage. Sie ist ein altbewährtes und aus heutiger Sicht unverzichtbares Therapeutikum das gegenüber den dermatologischen Therapien die am Wohnort durchgeführt werden können in der Regel nebenwirkungsärmer ist und insbesondere einen zusätzlichen Langzeiteffekt hat; dieser ist bei chronischen Krankheiten von besonderer Bedeutung und kann durch andere Therapieformen nicht erzielt werden. Damit stellt die dermatologische Klimatherapie eine ganzheitliche Therapieform dar und ist außerdem die umfassendste Therapie überhaupt. Eine dermatologische Klimatherapie kann nur in echten Reizklimazonen mit zusätzlich besonderen Insolations- thermisch-hygrischen und Aerosolverhältnissen durchgeführt werden. Durch diese klimatischen Gegebenheiten wird einerseits der Organismus allgemein vegetativ stabilisiert andererseits erfährt die erkrankte und konstitutionell veränderte Haut durch direkt wirkende Klimafaktoren einen günstigen Einfluß. Da letztlich jedes Klima eine spezifische Gesamtwirkung hat muß der therapeutische Sofort- und Langzeiteffekt eines Klimas durch klinische Verlaufsuntersuchungen wissenschaftlich belegt sein. Dermatological climatotherapy is used in the treatment of chronic and chronically relapsing longterm dermatoses such as especially atopic dermatitis and psoriasis complementing the dermatological therapies applied at the patient's place of living. It is a classical dermatological in-clinic therapy carried through in specific rough and stimulating climate areas of proven therapeutic benefit primarily the North Sea climate and certain alpine locations over 1 500 m above sea-level like especially in the high mountain valley of Davos Switzerland. Dermatological climatotherapy is a well-tried therapeutic agent which in comparison to the dermatological therapies applied at home as a rule has fewer side effects as well as above all an additional long-lasting time effect. The latter is of particular importance regarding chronical diseases and cannot be achieved by other therapies. Dermatological climatotherapy hence represents an all-entailing therapy form and moreover is the most comprehensive therapy anyway. Dermatological climatotherapy can only be performed in rough and stimulating climate zones with additional special insolation thermichygric and defined aerosol conditions. On the one hand the organism is stabilized general by those climatic conditions and on the other the constitutionally damaged skin is affected positively by the direct influence of climate factors. As every climate has its specific overall effect the therapeutical immediate and long-term effects of a climate have to be proven scientifically by follow-up studies.
