Abstract: A pharmaceutical speciality with a special extract from Kava-Kava rhizomes (root stocks) standardized at 70% Kava lactones has been subjected to comprehensive clinical trials over recent years. This special extract bears the code designation WS 1490.Human pharmacological studies confirm its cerebral bioavailability and the pharmacological activity of active substances in the extract (computer-assisted pharmacological EEG investigations). Studies in voluntary participants show that vigilance, concentration, mental retention, attentiveness and reaction capacity are in no way restricted. Furthermore, its combination with ethanol (serum alcohol level 0.5%0) shows no increase at all in alcohol-conditioned loss of reaction and concentration. On the contrary, there is evidence that WS 1490 increased reaction and concentration faculties in study participants.A number of placebo investigations and a reference-controlled study (involving tranquilizers) confirm the clinical efficacy of this Kava-Kava extract as an anxiolytic (on the level of symptoms and psychometrically). Both the 'psychological' and the IsomaticÂ’ forms of anxiety improve in a significant and clinically relevant manner. At the presently valid dosage of 100 mg extract 3 times per day (corresponding to 3 times 70 mg Kava lactones), it is frequently only a matter of a few days before patients suffering from anxious emotional instability already feel an effective level of improvement.Both in a postmarketing surveillance study and in a spontaneous report survey system, very rare gastrointestinal complaints and (allergic) skin reactions have been given as side effects. In ethnomedical reports, a yellowish coloring of the skin has been associated with the intake of Kava-Kava. No observations of this kind have been reported where the special extract WS 1490 is concerned. In patients suffering from conditions of nervous anxiety, tension and restlessness, this special extract from Kava-Kava is capable of being a useful alternative to chemical/synthetic pharmaceuticals.
