Abstract: Object: Estrogen replacement therapy (ERT) is a method of choise for preventing and treating postmenopausal osteoporosis. The potential side effects result in a controversy about its acceptance. Therefore, alternatives such as exercise deserve further investigations.Design: Prospective controlled study. Methods: Postmenopausal women were allocated into three groups: 47 women (mean [± SD] age 61.7 ± 5.8) performed an exercise program regularly; 38 women (mean [± SD] age 56.1 ±6.1) received estrogens; 52 (mean [± SD] age 58.1 ±6.8) served as controls and received no exercise or drug treatment. At entry and after 30 months singlephoton absorptiometry and biplanar roentgenograms of the thoracic and lumbar spine were taken. Results: After 30 months, significant bone loss (p < 0.05) occured in the control group, but not in the 2 treatment groups. Conclusion: It is concluded that both ERT and regular exercise may prevent bone loss in postmenopausal women.
