PRESSURE PAIN THRESHOLD (PPT) OF FIBROMYALGIA PATIENTS IS REDUCED AT TYPICAL TENDER POINTS AND NON TENDER POINTS . A PATIENT CONTROLLED AUTOMATIC PPT DEVICE IMPROVES MEASUREMENT ACCURACY.

Abstract: Klinik f. Physikalische Medizin Schmerzambulanz Klinikum der L.M.-Universität Muenchen-Großhadern Methods: PPT on 30 FM patients and 30 matched controls have been measured by two methods: a) by a manual DIGITAL DOLORIMETER 100 N (MUELLER); b) by a patient on/off controlled device with Linear Pressure Augmentation (LPA = 0 75 kg cm-2 s-1). In addition to 12 FM typical tender points (Lautenschlaeger) PPT was measured at 5 non tender points (fingernail middle phalanx PIP (all dig. III) m. add. pollicis and forearm flexors). All measurements have been repeated 5 times within 1 minute. Results: 1a) PPT was significantly reduced at all tender points: patients vs. controls maxim. -67 2 % (m. erect. trunci S1) minim. -36 7 % (m. masseter). 1 b) PPT of non tender points was similarily reduced: maxim. -43 5 % (forearm flexors) minim. -30 0 % (fingernail). 2) All PPT reductions were symmetrical. The dominant hand PPT was higher in both patients (18 %) and controls(8 %). 3) LPA measurement showed significant lower PPT than the manual method (overall mean ±SD for patients: 0 93 ± 0 2 / 1 43 ± 0 4 kg; for controls: 2 2 ± 0 2 / 2 76 ± 1 0 kg). 4) Measurement repeating had no influence on PPT... hl
