Course of rehabilitation in young aphasic stroke patients

Abstract: From a prospectively planned study to evaluate the course of rehabilitation in stroke patients a subset of 48 aphasic patients was investigated with reference to their logopedic, motoric and psychologic findings during a seven to eight week in-patient treatment. As a control group 156 nonaphasic hemiplegics with comparable demographie data were analysed with regard to their ability to perform the normal activities of daily life (Barthel-Index).The improvement of the speech disorder correlates significantly with an increase of the Barthel-Index. With respect to the "Aachener Aphasietest" the quantitatively most impressive changes are found with regard to the faults in the so-called "Token-Test" and the "Spontansprache". The disturbance of nonverbal functions accompanying stroke like perceptual performance and cognitive skills are directly related to the ability to perform the normal activities of daily life. We found aphasia to be an additional prognostic factor of stroke.A significantly lower level an the Barthel-Index, the Guttman-Scale and the "Nürnberger Altersfragebogen" a psychometric test was found in aphasic stroke patients when compared to those without a speech disorder. A multivariate statistical analysis indicates that the values of the Barthel-Index and the Guttman-Scale at the onset of the rehabilitation are valid determinants of the Barthel-Index after treatment. The improvement of the aphasia can not be predicted in a similar manner.
