Behandlung des Pleuraempyems mit proteolytischen Fermenten Eine Vergleichsstudie

Abstract: Dr. med. H. Lüllig Abteilung für Thorax-Chirurgie Zentralkrankenhaus Bremen Ost Zusammenfassung Fragestellung: Vergleich der Wirksamkeit und Verträglichkeit von instilliertem Plasmin/Desoxyribonuklease-Gemisch und Streptokinase/Streptodornase bei Patienten mit Pleuraempyem. Methode: Randomisierte einfachblinde Parallelstudie über 8 Tage. Patienten: Hospitalisierte Patienten mit einem Pleuraempyem unterschiedlicher Genese. Therapie: Tägliche Enzyminstillation und Spülung mit physiologischer Kochsalzlösung nach 3-4 Stunden. Endpunkte: Beschaffenheit der Drainageflüssigkeit und Röntgenbefund. Ergebnisse: Bei 14 von 18 Patienten unter Plasmin/Desoxyribonuklease und 12 von 18 Patienten unter Streptokinase/Streptodornase klärte sich die Drainageflüssigkeit von eitrig bis trüb zu serös und flockig. Röntgenologisch verkleinerte sich der Ausdehnungsgrad des Empyems in beiden Gruppen gleichermaßen. Schlußfolgerung: Hinsichtlich der Beschaffenheit der Drainageflüssigkeit und der Röntgenbefunde finden sich keine statistisch signifikanten Unterschiede zwischen beiden Prüfpräparaten. Treatment of Pleural Empyema with Proteolytic Enzymes. A Comparative Study: Study objective: Comparison of efficacy and tolerability of instilled plasmin/ desoxyribonuclease mixture vs. streptokinase/streptodornase in patients with thoracic empyema. Design: Randomized single-blind parallel study of a duration of 8 days. Patients: Hospitalized patients with a thoracic empyema of varying origin. Interventions: Once daily enzyme instillation followed by washing with physiological saline solution after 3-4 hours. End points: Condition of drainage liquid and X-ray findings. Main results: In 14 of the 18 patients under plasmin/ desoxyribonuclease therapy and 12 of the 18 patients treated with streptokinase/ streptodornase the drainage liquid cleared from purulent or cloudy to serous and flaky. In addition an equal reduction of the affected area is seen under X-ray. Conclusion: No statistically significant differences are noticed between the two trial preparations with regard to the condition of the drainage liquid and the X-ray findings. ab
