Lebensbedrohliche Myokardischämie ohne koronare Herzkrankheit

Heuser Medizinische Abteilung Schwerpunkt Kardiologie-Pneumologie Vinzenzkrankenhaus Hannover Zusammenfassung Koronarspasmen können auch bei Patienten ohne hämodynamisch wirksame Koronararterienstenosen zu schwerer Myokardischämie führen. Berichtet wird über zwei Patienten (55 bzw. 26 Jahre alt) mit unauffälligem Koronarangiogramm bei denen Koronarspasmen in Ruhe schwere teils atypische pektanginöse Beschwerden und komplexe ventrikuläre Arrhythmien bzw. einen Myokardinfarkt mit kardiogenem Schock auslösten. Therapeutisch sind im Akutstadium hochdosiert Nitroglycerin und Kalziumantagonisten (sublingual und parenteral) eventuell auch eine Thrombolyse indiziert; langfristig haben sich zur Prophylaxe am besten Kalziumantagonisten bewährt. Life-threatening Myocardial Ischemia without Coronary Artery Disease: Coronary artery spasm may induce severe myocardial ischemia in patients without coronary artery disease. Two patients 55 and 26 years of age with angiographically normal or nearly normal coronary arteries are reported in whom coronary artery spasm with resting angina induced complex ventricular arrhythmias in one and myocardial infarction with cardiogenic shock in the other. In the acute phase therapy consists of highly-dosed nitroglycerin and calcium antagonists sublingually and intravenously; in some cases systemic thrombolysis should be considered. For reducing the number of ischemic episodes calcium antagonists are the established long-term treatment. ab
