Lehrkrankenhaus Frankfurt/Main Zusammenfassung Sowohl für den Arzt als auch für den unheilbaren Patienten und seine Angehörigen werden in den Bereichen: Arztverhalten Aufklärung und Psychotherapie Kriterien herausgearbeitet; diese sollen helfen den Umgang zu verbessern und zu erleichtern. 1. Die häufig unbewußten Reaktionsformen des Therapeuten in Form von Überaktivität Rückzug Ersatzhandlung und Ärger gilt es bewußt zu machen. 2. Die Frage der Gefahren und des Für und Wider bei der Aufklärung wird detailliert an den Patientenmerkmalen: Persönlichkeitsstruktur soziale Einbindung Lebensalter Religiosität und Dauer des Wissens um die eigene Krankheit systematisch an 56 bereits aufgeklärten chronisch unheilbaren Kranken untersucht. Als Resultat konnten Hauptkriterien gefunden werden die bei jedem Einzelfall zu beachten sind. 3. Für die psychotherapeutische Begleitung des Patienten werden Handlungsmöglichkeiten abgeleitet wie z. B. antizipatorische Trauerarbeit und "Überstieg". Summary For the therapist the terminally ill patient and his relatives criteria are investigated in three dimensions: therapist's behavior disclosing the diagnosis and psychotherapy for the purpose to improve the communication. 1. For the therapist it is necessary to become aware of his unconscious reactions such as overactivity withdrawal substitute act and anger. 2. The danger of disclosing the diagnosis and the pros and cons are systematically investigated with the help of the features: personality structure social contact age religiousness and time factor; applied to 56 terminally ill patients who were already informed about their condition. The result was the finding of main criteria which have to be considered in each individual case. 3. For the psychotherapeutic support of the patient possibilities to cope with the condition are shown for example anticipating mourning work and a special kind of identification. wt
