Die Therapie NSAR-induzierter gastrointestinaler Läsionen mit Misoprostol

Bach) Bayreuth Zusammenfassung: In einer multizentrischen placebokontrollierten Doppelblindstudie bei Patienten mit degenerativem oder entzündlichem Rheumatismus wurde das Prostaglandinanalogon Misoprostol in einer Dosierung von 2x200 µg postprandial zur Therapie NSAR-induzierter Läsionen und Symptome des oberen Gastrointestinaltraktes verabreicht. Die antirheumatische Therapie mit einem NSAR wurde dabei fortgeführt. Die Behandlung mit Misoprostol erwies sich gegenüber Placebo als deutlich überlegen in bezug auf die Verminderung gastroduodenaler Läsionen und gastrointestinaler Schmerzen. Ebenso fiel die ärztliche Globalbeurteilung gastrointestinaler Beschwerden in der Misoprostolgruppe statistisch signifikant besser aus. Die antirheumatische Wirkung der NSAR war in beiden Therapiegruppen nicht zu unterscheiden. Summary: In a multicenter placebo-controlled double-blind study in patients with degenerative or inflammatory rheumatic diseases the prostaglandin analogue misoprostol in a dose of 200 µg twice daily after meals was administered for the treatment of NSAID-induced lesions and symptoms of the upper gastrointestinal tract. Treatment with an NSAID was continued. Misoprostol treatment was demonstrated to be clearly superior to placebo in respect of reduction of gastroduodenal lesions and gastrointestinal pain. In addition the physcian's global assessment of gastrointestinal complaints was also statistically significantly better.
