Der chronische Kopfschmerzpatient im Rahmen einer neurologischen Schmerzambulanz - Diagnostische und therapeutische Überlegungen -

Abstract: In medicine cephalalgy is the most frequent pain symptomatology. The basis of every treatment of the cephalaigy is a thorough clarification of the symptomatology in order to exclude symptomatic forms of headache based an Organ-pathological findings. The treatment of syndromes of chronic headache is increasingly performed in specialized institutions with treatment as outpatient or inpatient. Based an the experience of a neurological unit for ambulatory treatment of pain it is reported an the diagnostic proceeding in case of chronic headache, the classification of the forms of headache according to the classification of the IHS as well as therapeutic principles.Part of the patients turns out to be resistant to therapy and that in spite of intensive therapy. In the patients that are resistant to therapy (nonrespondents) both the long-term administration of medicaments and also factors arising from the psychosocial field seem to have an especially chronicifying effect. Therefore, for the practice it is important, besides the exact clinical classification of the headache syndrome, to determine the extent of chronicifying factors already before the beginning of the therapy and to perform a multidimensional therapy in these patients. In such a structured cephalalgy therapy also nonmedicinal portions of therapy have to be sufficiently considered besides the medicinal treatment.
