Nisoldipin im Vergleich zu Nifedipin bei stabiler Angina pectoris Wirksamkeit und Verträglichkeit von 1 x 10 mg Nisoldipin vs. 3 x 10 mg Nifedipin

Abstract: Dr. med. K. Heß Hannover Zusammenfassung Fragestellung: Gibt es Unterschiede zwischen einer antianginösen Dauertherapie mit Nisoldipin und Nifedipin im Hinblick auf Wirksamkeit und/oder Verträglichkeit? Methode: EKG-kontrollierte Fahrradergometrie Angina-pectoris-Häufigkeit Nitrat-Verbrauch Routinelabor Nebenwirkungserfassung. Patienten: 54 Patienten mit stabiler Angina pectoris. Therapie: Nisoldipin (1 x 10 mg) bzw. Nifedipin (3 x 10 mg) über 6 Monate. Ergebnisse: Beide Präparate führten im Verlauf der 6monatigen Therapie zu einer signifikanten Steigerung der ergometrischen Belastbarkeit (Nisoldipin: von 87 auf 109 Watt; Nifedipin: von 86 auf 101 Watt) sowie der Zeit bis zum Auftreten einer ST-Strecken-Senkung von 0 1 mV (Nisoldipin: von 63 auf 171 Sekunden; Nifedipin: von 62 auf 165 Sekunden). Beide Therapien reduzierten die Häufigkeit von Angina-pectoris-Anfällen (Nisoldipin: von 8 auf 1 pro Woche; Nifedipin: von 9 auf 3 pro Woche) bzw. den Nitrat-Verbrauch (Nisoldipin: von 7 auf 1 Einheit pro Woche; Nifedipin: von 11 auf 3 Einheiten pro Woche). Bei je 7 Patienten beider Behandlungsgruppen traten Nebenwirkungen auf. Keine Änderungen wurden bei den Laborparametern beobachtet. Schlußfolgerung: Nisoldipin und Nifedipin sind äquieffektiv und gleich gut verträglich. Nisoldipin besitzt jedoch den Vorteil der Einmaldosierung. Nisoldipine in Comparison to Nifedipine in Patients with Chronic Stable Angina Pectoris. Efficacy and Tolerability of 1 x 10 mg Nisoldipine vs. 3 x 10 mg Nifedipine: Study objective: Are there any differences between long-term antianginal treatment with nisoldipine and with nifedipine with respect to efficacy and/or tolerance? Method: Electrocardiographic stress testing on bicycle ergometer frequency of anginal attacks nitrate consumption routine laboratory tests documentation of side-effects. Patients: 54 patients with stable angina pectoris. Interventions: Nisoldipine (10 mg once daily) and nifedipine (3 x 10 mg) respectively over 6 months. Main results: Over the 6-month treatment period both preparations led to a significant increase in maximal workloads in the stress testing (nisoldipine: from 87 to 109 watts; nifedipine: from 86 to 101 watts) and prolonged the time before appearance of an ST-segment depression of 0.1 mV (nisoldipine: from 63 to 171 seconds; nifedipine: from 62 to 165 seconds). Both treatments reduced the frequency of anginal attacks (nisoldipine: from 8 to 1 per week; nifedipine: from 9 to 3 per week) and the nitrate consumption (nisoldipine: from 7 to 1 unit per week; nifedipine: from 11 to 3 units per week). Side effects occurred in 7 patients in each treatment group. No changes in laboratory parameters were observed. Conclusion: Nisoldipine and nifedipine are equieffective and equally well tolerated.
