Profil eines neuen Antihypertensivums: Ketanserin

Bühler Departement Forschung und Medizin Kardiologische Abteilung Kantonsspital Basel Zusammenfassung Dem Serotonin kommt im älteren atherosklerotisch geschädigten Kreislaufsystem eine zunehmende pathophysiologische Bedeutung zu da es infolge einer veränderten Kinetik sowie einer Schädigung des Gefäßendothels eine verstärkte Vasokonstriktion und Thrombozytenaggregation bewirkt. Neue therapeutische Möglichkeiten eröffnet der S2-Rezeptor-Antagonist Ketanserin der über eine Blockade der Serotoninwirkung an den glatten Gefäßmuskelzellen und an den Thrombozyten antihypertensiv und aggregationshemmend wirkt. Die Blutdrucksenkung durch Ketanserin erfolgt über eine Reduktion des erhöhten peripheren Widerstandes. In einer Dosierung von 2 x 20 mg bis 2 x 40 mg täglich zeigt Ketanserin eine gleich starke antihypertensive Wirkung wie Diuretika Betablocker Kalziumantagonisten und ACE-Hemmer. Ketanserin eignet sich besonders zur Behandlung des älteren Patienten mit hohem Blutdruck - aufgrund einer besseren Wirksamkeit und Verträglichkeit im Alter sowie wahrscheinlich einer zusätzlichen inhibitorischen Wirkung auf die Thrombozytenaktivität. Die in pharmakologischen Modellen an der Koronararterie des Hundes ermittelte antithrombotische gefäßprotektive Wirkung von Ketanserin bedarf noch weiterer Bestätigungen. Profile of a New Antihypertensive Agent: Ketanserin. Serotonin plays a pathophysiological role particularly in the ageing cardiovascular system with atherosclerotic lesions where derangements in serotonin kinetics and endothelial damage promote an excessive vasoconstriction and platelet aggregation. The S2-receptor antagonist ketanserin offers new therapeutic possibilities by preventing the action of serotonin on the vascular smooth muscle and on the platelet which results in decreasing blood pressure and in platelet deactivation. Ketanserin exerts its antihypertensive action by a reduction in total peripheral resistance. The antihypertensive efficacy of ketanserin at a dose of 2 x 20 mg to 2 x 40 mg daily is comparable to that of diuretics beta blockers calcium antagonists and ACE-inhibitors. Ketanserin is particularly recommended for the treatment of the elderly hypertensive patient because of a better efficacy and tolerability in older age and probably an additonal inhibitory action on platelet activity. The antithrombotic vasoprotective action of ketanserin demonstrated with pharmacological models on the coronary artery of the dog has to be confirmed yet by further studies. ab
