Antibiotikainduzierte Störungen der aeroben Darmflora

Abstract: Anti biotic-induced disturbances of the aerobic intestinal microflora: changes in flora composition by selective antimicrobial decontamination in immunocompromised patients. In this prospective clinical surveillance study of the aerobic intestinal flora, the effect of selective decontamination with cotrimoxazole and ketoconazole is examined by determination of quality, quantity and antibiogram of the isolated microorganisms. The desired elimination of potentially pathogenic microorganisms by continuous decontamination results in the generation of a resistant autochthonous flora as well as in the acquisition of multi-resistant hospital strains, and finally in a dramatic increase of pathogenic yeasts in the intestine. In order to evaluate the efficacy of the decontamination procedure an analysis of fecal flora concerning enterobacteriaceae, pseudomonadaceae and yeasts is required from the second week on, in order to timely adapt the antibiotic regimen, if necessary.
